In order to attend our Disciple Now on July 31, 2009 through August 2, 2009, you MUST fill out and have a parent sign the registration form.  You are welcome to download and fill out this form online and then print it out.  Once a parent has signed the form, please turn it in to the office with your registration fee. REMINDER: Every student will need an up-to-date, signed, and NOTARIZED medical release and permission form on top of the registration form.  You can find those documents on this website as well, under “FORMS.”

Hernando Parks and Recreation is starting an Ultimate Frisbee League.  If you are 16 or older and interested in playing to need to have a parent fill out the form below. We will be taking the players from DeSoto Hills Baptist Church and combining with players from New Prospect Baptist Church to make a full team.  Forms are due by Monday, December 15.  The league will by played on Monday nights at the old Hernando High School football field.  Games start in mid-Jan. and run through mid.-March.  The cost per person is $25.00 and each player will receive a t-shirt and frisbee.  If you have any questions contact Michael… (901)-605-9083

Parents & Students, Every year a survey is released that allows students and parents to express all their hopes and dreams for the student ministry of their church.
