Disciple Now 2009 Registration Form
June 30th, 2009In order to attend our Disciple Now on July 31, 2009 through August 2, 2009, you MUST fill out and have a parent sign the registration form. You are welcome to download and fill out this form online and then print it out. Once a parent has signed the form, please turn it in to the office with your registration fee.
REMINDER: Every student will need an up-to-date, signed, and NOTARIZED medical release and permission form on top of the registration form. You can find those documents on this website as well, under “FORMS.”
